OSHA Investigating Death of Boy, 15, in Forklift Accident

The federal Occupational Safety and Health Administration on Tuesday began investigating the death of a boy who was pinned beneath a forklift at a warehouse.
Martin Salgado, 15, of Santa Ana was driving the forklift about 8:15 p.m. Monday at Towne Advertiser’s Mailing Service, 511 Goetz Ave., when it overturned and landed on him, police spokeswoman Maureen Thomas said.
Salgado’s co-workers freed him with a second forklift and called paramedics. He died at Western Medical Center at 9:20 p.m. of head and chest injuries.
Towne Advertising executives could not be reached for comment.
